Welcome to Bridge House Medical Centre and thank you for choosing to register with us.
Bridge House Medical Centre provides personal, safe and accessible general practice, including our dispensary, to the people of Stratford-upon-Avon and the surrounding villages.
We do this through a stable team who know their patients, care grounded in evidence, and a habit of measuring what we do and making it better. For more about our mission, click here.
Our practice consists of 3 GP partners, 4 salaried GPs, 3 Registrars, 4 Maternity cover GPs, 2 Advanced Nurse Practitioners, 2 Practice Nurses, 1 Healthcare Assistant, 4 Dispensers, 5 Reception staff, 2 Secretaries, and 8 admin team staff representing around 9,000 patients.
Within practice we house 10 Clinic rooms and 3 Treatment/Exam rooms. Based on Scholars Road, we are within walking distance of the town centre’s bus stops, Stratford-upon-Avon Train Station, and Stratford Hospital.
We offer a wide range of clinics and services from Minor Operations to the Cryotherapy Clinics. We offer specialist clinical support within practice as well as a wide range of physical and mental wellbeing support through our PCN.
Appointment Schedule
Our GP appointments are accessible through the total triage system via our website or this link. Once a triage is submitted, the on call GP will review and will respond to you within 4 hours for urgent requests and on the same working day for routine requests. They will signpost you to the most appropriate service, this could be a face to face or telephone appointment with a clinical pharmacist, first contact physiotherapist, an appointment with a GP and so forth.
For our vulnerable patients that do not have internet access, reception will be able to fill this out over the phone (01789 292137) or in person.
Prescriptions
When you register with the practice, we receive your record from your previous GP practice. However, in order to ensure you can reorder your prescriptions for the next month, we need you to have a discussion with one of our Clinical Pharmacists/GP and for you to provide proof of your prescription list.
Once this is done your prescriptions will be set up with the practice, including your nominated pharmacy.
Online Services
As a practice, we want to empower you to have more control over your healthcare. One of the best ways you can achieve this is by ensuring you are utilising the NHS App or Patient Access app to its full extent. Both of these apps can be used to order repeat prescriptions, view your health record and keep up to date with hospital appointments.
In order to set this up we do require a form to be filled in within practice. If you need further help to get you set up, you can come and see our digital care coordinator, who will introduce you to the app and ensure you are confident in using all of its features
Long Term Health Conditions and Medication Reviews
LTCs and Annual Review systems: If you suffer with any long term health conditions (LTCs) or take any medication, we do require you to have an annual review. Depending on your condition and management plan, this could be more frequent that annually. In order to ensure this, we have set up a review system which will recall you during your birth month.
Any blood tests or monitoring checks that are required will be completed the month prior to your birth month, followed by your review with the relevant clinician in your birth month. If, at any time, you believe you need a review for your LTC, this can also be arranged.
