Annual Reviews
If you have a long-term condition, you will be invited for your annual review on your month of birth. You will be informed within your invite if you need to fill out an online questionnaire, if you need a face-to-face appointment and if you need to have blood taken before your appointment.
Travel Vaccinations and Non-NHS Fees
If you require any vaccinations relating to foreign travel, you will need to make a telephone appointment with a nurse. This initial appointment must be at least 6 weeks before you travel.
Before attending your first appointment, please complete this Travel Questionnaire and submit it.
The NHS does not pay for all of the services we provide, so patients may be required to pay for some of these services. Please see attached list.
Our fees are in line with BMA approved national guidelines.
NHS Health Check
The NHS Health Check is for patients that are ages 40-74 who do not have any pre-existing Chronic conditions. To view the list of conditions, please click here.

Flu Clinics
For all patients that are entitled to their free annual flu vaccine, they will be invited via text, telephone call or letter.

NHS Abdominal Aortic Aneurysm (AAA) Screening Programme
This is a free NHS AAA scan for males aged over 65. Men are automatically invited for screening in the year they turn 65. Men who are older than 65, and who have not previously been screened or treated for an abdominal aortic aneurysm, can opt in through self-referral direct to the screening programme. Men receive an invitation leaflet with an appointment time three weeks in advance. If accepts the invitation, an ultrasound scan of the abdomen is carried out and the aortic diameter measured and results are provided verbally immediately after the scan and in the post shortly afterwards.

Diabetic Eye Screening Portal
The Diabetic Eye Screening service now have a dedicated portal for its patients.
If you are under their care, you can now use the portal to book your next appointment, cancel or rebook an existing appointment, change location of appointment, contact preferences and view your results. These results include screening images from past appointments. Please visit the portal via – https://portal.ahwdesp.co.uk .
Over 75 Health Check
All eligible patients, who are over the age of 75, will be invited via text, telephone call or letter.
